How to make a referral

Individuals being referred must meet the following criteria:

  • homeless or at risk of becoming homeless
  • alcohol dependent
  • history of non-engagement.

All referrals must include the named Care Manager.

Referrals must be sent to Service Commissioning Officer, Ellen McPhilemy, who will advise if the referral is suitable.

If a referral is suitable, a Health and Wellbeing Team Leader will contact the relevant Care Manager to arrange a service visit followed by a pre-planning meeting.

A Health and Wellbeing Team Leader and Senior Housing Administrator will then agree a moving-in date.
